At our first district meeting of the 2002 year, Saad
E. Shaban presented "Assessment of the Sacrum". Saad works
at Golden Hills Orthopedic and Sports Physical Therapy Inc. He has
been a physical therapist for 10 years. He graduated from the Cairo
Medical School and the Texas Women's University in Houston, Texas. He
is presently in his fifth year of osteopathic study at the College
d'Etudes Osteopathiques in Montreal, Canada.
Saad started off his presentation with some anatomical
review of the pelvic anatomy, including muscles, bones, viscera,
nerves and foramen. He then specifically described the anatomy of the
sacrum. Saad reminded us that the sacrum moves on 4 axis: sagittal,
coronal and 2 oblique. This is very important to remember when
assessing function and dysfunction of sacral movement.
When assessing the sacrum, its position and movement
are evaluated in a variety of patient postures and movements. The
sacrum is evaluated in sitting, standing, prone and supine. The
patient then performs flexion/extension, side bending and rotation in
these positions while the therapist evaluates the sacrum's position
and mobility. The therapist does this by comparing the left and right
positions of the sacral bases and inferior lateral sacral edges, before/during
/after the patients movement. The sacrum's relative position to both
iliac is also evaluated. Identification of the sacrum's position and
movement will identify and differentiate between a sacral dysfunction
and an iliac dysfunction.
Saad was not able to go through treatment strategies
for the different sacral and iliac dysfunctions due to time
limitations, but was invited to return to a future district meeting
to complete his presentation.
